First time user, please register: click Register and complete all the fields.  Please enter your MyCanyons email as the username, then your Student ID, your My Canyons email, First and Last Name, then hit submit.

First time users after signing up, please go to your My Canyons email, you will find an email that has a link to create a password.

Password requirements: one uppercase, one lowercase, one special character with a mix of number and letters.

Once your password is set, please login to the Patient Portal and hit the Forms Button.  Complete the appropriate paperwork for your appointment.
